Practisphere provides comprehensive healthcare management services including:

• Multi-specialty healthcare treatment and care coordination
• Appointment scheduling and telehealth services
• Prescription management and e-prescribing
• Billing and insurance verification services
• AI-assisted clinical documentation (with provider review)
• Patient portal access to health records

User Responsibilities

You are responsible for:

• Maintaining the confidentiality of your account credentials
• Providing accurate and complete information
• Complying with all applicable laws and regulations
• Notifying us of any unauthorized use of your account

Privacy and Confidentiality

We are committed to protecting your privacy and maintaining the confidentiality of your protected health information (PHI) in accordance with HIPAA regulations. Please review our Privacy Policy for detailed information.

Electronic Signatures

By using our electronic consent and signature features, you agree that your electronic signature is legally binding and equivalent to a handwritten signature under the ESIGN Act (15 U.S.C. § 7001). You may request paper copies of any documents you have signed electronically.

Payment Terms

Payment for services is due at the time of service unless other arrangements have been made. We accept various payment methods and work with most insurance providers. For uninsured or self-pay patients, a Good Faith Estimate of expected charges will be provided in advance per the No Surprises Act.

Limitation of Liability

Practisphere shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use or inability to use the service.

Changes to Terms

We reserve the right to modify these terms at any time. Continued use of our services constitutes acceptance of any changes.
